Pry out the round bolt cover on the outside of the door using a flathead screwdriver in the notch for it. Pry out the upper interior cover to release it with same screwdriver. Then use a socket to remove the bolt. (There is also a small phillips/10 mm hex driver bolt near the top that you can access from outside the car that holds the interior panel in place that needs to be removed if you are going to remove the entire interior door panel.
Tip: once you have it removed use a piece of masking tape to hold all the associated washers, etc. to the bolt so you keep them in the same order.
